Assessment and Referral

Assessment is tailored to meet an individual’s needs and the assessment duration will vary. The first part of the process is attendance at an information session at Palmerston Perth that gives an overview of the Farm program and provides potential residents with the opportunity to ask any questions they may have about the program. Upon completion of an initial assessment interview, which the client arranges after the information session, and receipt of medical forms, clients progress to a Farm day visit. Following this, if it is agreed that the program meets the client’s needs, the client is booked to enter the Farm program depending on available bed spaces.
