Working with GP Down South team in the Peel Health Hub

 

 

The Palmerston Clinical Care Coordination service is a pilot aimed at supporting young people under the age of 25 through a model of interagency coordinated care in the Peel region.

Palmerston Clinical care coordinators work as part of the existing GP Down South Care Coordination team in the Peel Health Hub providing direct services to young people through a formalised model of holistic and shared care delivery involving a range of other agencies. These positions ensure continuity of care from assessment through to discharge/referral and are responsible for effective communication between services to support positive client outcomes.

 

As a Clinical Care Coordinator you will be experienced working with youth in AOD/ mental health related counselling as well as case management of young clients presenting with complex and co-occurring needs.  You should have demonstrated success working in partnership with multidisciplinary clinical services along with well-developed case reporting and record keeping skills.

 

This position is offered as a fixed term position ending 30th November 2022.  Salaries are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010. Level 5, pay point commensurate with experience, qualifications and role.
